For the first week of illness, you should fill in a ‘self certificate’.

If you are employed, you need form SC2 which you can get from your employer or downloadable HERE.

If you are self employed, disabled or unemployed, you need form SC1. (Incapacity Benefit Form). These are available at the surgery or can be collected from post offices and DSS offices.

If you want a private certificate for the first few days of illness these are available but there is a charge for them. Do you really need it?

After the first week, you will need to contact the doctor for a further certificate.
